These Terms of Service ("Terms") constitute a legal agreement between you ("you," "your") and Slips LLC ("Form," "we," "us," or "our") and govern your access to and use of the Form mobile application (the "App") and all related services provided by Slips LLC (collectively, the "Services"). By downloading, using, or accessing the Services, you agree to be bound by these Terms and our Privacy Policy.

If you do not agree to these Terms, do not use the Services.

1. Eligibility

You must be at least 13 years old to use the Services.
If you are under 18, you must have parental or guardian consent.
You must not use the Services if doing so violates applicable laws or regulations.

2. Account Registration

You agree to provide accurate, current, and complete information during the registration process.
You are responsible for maintaining the confidentiality of your account credentials and for all activities under your account.
You may delete your account at any time through the App.

3. License to Use

We grant you a limited, non-exclusive, non-transferable, revocable license to use the Services for personal, non-commercial purposes. You agree not to:

Resell or distribute the Services
Reverse engineer or modify any part of the Services
Use the Services in violation of applicable laws or regulations
Upload or distribute offensive, harmful, or infringing content

4. Health Disclaimer

Form is not a medical or healthcare service and does not offer medical advice. You should consult your physician before starting any exercise or wellness program. Use of the Services is at your own risk.

5. Payments and Subscriptions

In-app purchases are processed by Apple or Google. We do not store or handle payment information.
Some features require a subscription. Subscriptions renew automatically unless canceled.

6. Third-Party Services

The Services integrate with third-party tools including:

Supabase (data storage and authentication)
Mixpanel (analytics)
Sentry (crash reporting)
OneSignal (notifications)
Superwall (paywall and monetization)

Use of these tools is governed by their respective terms and privacy policies.

7. Modifications to the Services

We reserve the right to modify or discontinue any part of the Services without notice. We are not liable for any modification, suspension, or discontinuation.

8. Intellectual Property

All content, branding, and materials within the Services are owned by Slips LLC or our licensors. You may not use, reproduce, or distribute any content from the Services without our prior written consent.

9. Feedback

If you provide feedback or suggestions to us, you grant us a perpetual, royalty-free, worldwide license to use, modify, and incorporate it into our Services without any obligation to you.

10. DMCA and IP Infringement Policy

If you believe content within the App infringes your intellectual property, please contact support@useformapp.com with:

Your contact info
Description of the work being infringed
Description of where the infringing material is located
A statement under penalty of perjury that your claim is accurate

We reserve the right to remove infringing content and terminate repeat infringers.

11. HIPAA Disclaimer

Form is not a HIPAA-compliant platform. Do not upload, store, or share protected health information (PHI) through the Services. We disclaim any responsibility for the handling of PHI.

12. Export Compliance

You agree not to use or access the Services if you are on any U.S. government list of prohibited or restricted parties or are located in a country subject to a U.S. government embargo.

13. Termination

We reserve the right to suspend or terminate your access to the Services at our sole discretion, including if you violate these Terms.

14. Disclaimers

The Services are provided "as is" without warranties of any kind. We do not guarantee the accuracy, availability, or reliability of the Services. You use the Services at your own risk.

15. Limitation of Liability

To the fullest extent permitted by law, Slips LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of profits or revenues.

16. Indemnification

You agree to indemnify and hold harmless Slips LLC and its affiliates from any claims, damages, liabilities, and expenses arising from your use of the Services or your violation of these Terms.

17. Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of New York. Any disputes must be resolved in the state or federal courts located in New York County, NY. You waive your right to participate in class actions and agree to resolve disputes on an individual basis.

18. Changes to These Terms

We may update these Terms from time to time. When we do, we will revise the "Last Updated" date. Your continued use of the Services constitutes acceptance of the new Terms.
